If you file a public report: You will be using the OGE Form 278e (Public Financial Disclosure Report). This form may be found on OGE's web site. (Link to OGE Form 278e on OGE's web site)
You are also required to file an OGE Form 278-T (Periodic Transaction Report) for certain reportable transactions.
Your Agency may also require that you file this form in Integrity, an electronic financial disclosure system created by the U.S. Office of Government Ethics.
If you file a confidential report: You must initially submit a New Entrant OGE Form 450 as part of your appointment. You will then annually file another New Entrant OGE Form 450 UNLESS your HHS division has an alternative OGE-approved supplemental form to use in lieu of the OGE Form 450. As always, check with your HHS ethics officials; or committee management official (CMO); or Designated Federal Official (DFO) to find out which form you must submit. Send the completed report to the appropriate ethics official within your agency (not to OGE).
