The OGE Form 201 is used to request copies of the public financial disclosure reports and other specified records of certain executive branch officials. These records are available under the Ethics in Government Act of 1978 ("the Act") and amendments thereto (5 U.S.C. app. 103, 105 and 402), the STOCK Act, and OGE's implementing regulations. The Act imposes restrictions on the use of the records. They cannot be used for:
- any unlawful purpose;
- any commercial purpose, except by news and communications media for dissemination to the general public;
- determining or establishing a credit rating; or
- direct or indirect use in the solicitation of money for any purpose, including political and charitable purposes.

Copies of the certified public financial disclosure reports (OGE Form 278e and OGE Form 278-T) and, as appropriate, Certificates of Divestiture may be obtained from NIH by completing the online OGE Form 201, Request to Inspect or Receive Copies of Executive Branch Personnel Public Financial Disclosure Reports or Other Covered Records.
